Abstract
Continuous renal replacement therapy (CRRT) is a complex, high-risk intervention used in intensive care units (ICUs) for critically ill patients with acute kidney injury and hemodynamic instability. ICU nurses are responsible for initiating, monitoring, troubleshooting, and discontinuing CRRT while providing continuous bedside care (Richardson & Whatmore, 2015). However, variability in training frequency and competency validation remains a significant gap, potentially contributing to errors and adverse patient outcomes. Guided by the Iowa Model of Evidence-Based Practice, a literature review was conducted to identify best practices for CRRT training. A proposed evidence-based practice (EBP) project was developed to implement a standardized CRRT training program aligned with current literature. Nurse competency was evaluated using the QSEN competencies domains of knowledge, skills, and attitudes. Findings indicate that multimodal training is the most effective approach for CRRT competency. Optimal training includes didactic education, hands-on practice, simulation, and supervised clinical experience with a CRRT expert or super user. Training outcomes are assessed through quizzes on CRRT understanding, nurses giving demonstrations on troubleshooting alarms and setting up treatments, and self-surveys on attitudes toward CRRT patient care and confidence in skills. Implementing a comprehensive, multimodal training program with ongoing annual education improves nurse competency, enhances patient outcomes, reduces therapy interruptions, and decreases equipment-related costs.
